چکیده

5-Aminolevulinic acid dehydratase (ALAD) activity in two patients with compound heterozygous 5-aminolevulinic acid dehydratase deficiency porphyria was studied over the last 20 years. The patients' enzyme activity was <10% from 1977 to 1997. An acute crisis in each patient was successfully treated by infusion of glucose and heme arginate. After this therapy both urinary 5-aminolevulinic acid (ALA) and total porphyrins were diminished to 65% in patient B. In patient H, ALA was decreased to 80%, and total porphyrins were reduced to 15% after treatment with heme arginate and glucose. The patients remained free of symptoms after this therapy. Family studies of patient B showed cross-reactive immunological material (CRIM), in which the maternal mutation is CRIM(+), whereas the paternal mutation is CRIM(-). Incubation of erythrocyte lysates with ALA decreased porphyrin formation, whereas incubation with porphobilinogen produced porphyrin concentrations within reference values in both patients, confirming that ALAD activity is rate-limiting in these cells.

Clinically Important Features of Porphyrin and Heme Metabolism and the Porphyrias

Heme, like chlorophyll, is a primordial molecule and is one of the fundamental pigments of life. Disorders of normal heme synthesis may cause human diseases, including certain anemias (X-linked sideroblastic anemias) and porphyrias. Porphyrias are classified as hepatic and erythropoietic porphyrias based on the organ system in which heme precursors (5-aminolevulinic acid (ALA), porphobilinogen ...

Aminolaevulinate dehydratase porphyria in infancy. A clinical and biochemical study.

Homozygous deficiency of aminolaevulinate dehydratase (porphobilinogen synthase, EC 4.2.1.24) was diagnosed in a small child. The clinical presentation was unique since severe symptoms were already present in the neonatal period. The patient, a boy, now three-years old, had recurrent attacks of pain, vomiting, hyponatraemia and symptoms of polyneuropathy engaging motor functions including respi...

Circadian rhythms in acute intermittent porphyria--a pilot study.

BACKGROUND Acute intermittent porphyria (AIP) is an inherited disorder of haem synthesis wherein a partial deficiency of porphobilinogen (PBG) deaminase (PBGD) with other factors may give rise to biochemical and clinical manifestations of disease. The biochemical hallmarks of active AIP are relative hepatic haem deficiency and uncontrolled up-regulation of hepatic 5-aminolevulinic acid (ALA) sy...

More on identifying inherited deficiency of porphobilinogen synthase.

Tishler and Winston recently described a screening test for delta-aminolevulinic acid in urine (1). The test is said to be useful for identification of an inherited deficiency of delta-aminolevulinic acid dehydratase (porphobilinogen synthase, EC 4.2.1.24). To complete the picture regarding this recently described inheritable biochemical lesion, we wish to note that the patients described by Do...

Regulation of heme biosynthesis in Neurspora crassa.

The mold Neurospora crassa does not accumulate porphyrins in iron deficiency but instead accumulates the sideramine desferricoprogen. In iron deficiency there is an accumulation of &aminolevulinic acid and the d-aminolevulinic acid dehydratase level is very low. A similar situation exists in cobalt toxicity and zinc deficiency. The b-aminolevulinic acid synthetase and ferroprotoporphyrin chelat...
